Registered Nurse, Orthopaedic Certificate.

Position:
Discharge Planning Nurse.

Interests in the WJRC:
Participate in the small group presentations organized for the Joint Replacement Patients pre operatively. Conduct Individual discharge planning assessments when the patients attend the Pre Admission Clinic. Post discharge follow up by the Primary Health Nurse (community nurse), and the patients plans for post discharge are discussed at this time. Attend referrals to Primary Health Nurses when patients are discharged. Act as a contact/resource person for The Spastic Centre for patients with Cerebral Palsy who are undergoing Joint Replacement Surgery.
